Case Manager
Essential
Duties and Responsibilities:
· Provide
case management to assigned clients by identifying needs and barriers,
coordinating appropriate services and referrals, and following up on client
progress.
· Develop
and review individualized reentry plans with clients. Document goals, action
steps, progress, and updates as client needs or circumstances change.
· Provide
supportive counseling and client evaluations within the employee's
qualifications and assigned scope. Refer concerns requiring additional services
to appropriate staff or providers.
· Maintain
client schedules, verify employment, and complete fee collection records.
Document changes and report discrepancies or concerns to the appropriate
supervisor.
· Perform
facility head counts, rounds, client accountability checks, and safety
monitoring. Document and promptly report safety concerns or changes in client
status according to program procedures.
· Monitor
client self-administration of medication within assigned responsibilities,
maintain required documentation, and report medication-related concerns to
appropriate staff.
· Maintain
accurate and timely service records, communicate professionally with clients
and authorized partners, and protect confidential information in accordance
with agency procedures.
· Follow Department of Correction contractual requirements, the Residential Provider Manual, and agency policies. Complete required training, maintain professional boundaries, and seek supervisory guidance when needed.
Required Knowledge, Skills, Abilities and Other Characteristics:
Knowledge Of:
· Electronic health
record systems.
· General office
software, including Microsoft Office applications.
Ability To:
· Work effectively
with representatives of funding sources and referral agencies.
· Analyze problems
and identify effective, timely solutions.
· Communicate
clearly and professionally, both verbally and in writing.
· Organize
priorities, manage multiple responsibilities, and meet deadlines.
· Work
independently and exercise sound judgment.
· Learn and apply
new information, systems, and technology.
Education and
Experience:
· Bachelor's degree
in counseling, criminal justice, or a related field.
· Experience
working in a criminal justice setting.
· Successful
completion of introductory employment requirements.
· Demonstrate
ethical conduct, initiative, reliability, and attention to detail.
· Maintain
effective working relationships with clients, staff, and management.
· Perform routine
office duties, including climbing stairs, carrying objects weighing up to 20
pounds, and operating computers and office equipment, with or without
reasonable accommodation.
· Possess a valid
driver's license.

Pay Range
Pay Range:
$21.00-$23.10 per hour. Compensation is based on our established pay scale.
Starting pay within this range is determined by the position, with
consideration given to applicable licensure, certifications, and relevant
experience.
