Case Manager
Essential Duties & Responsibilities
? Perform and document all case-management functions, including reentry plans and reviews, progress notes, fee collection, client schedules, employment verifications, counseling, and evaluations.
? Provide direct client-care services such as facility headcounts, tours, monitoring self-administered medications, accountability checks, and ensuring facility and grounds safety.
? Complete all mandated training.
? Knowledge of and understanding of all Department of Corrections contractual agreements in accordance with the Residential Provider Manual.
Knowledge, Skills & Abilities
? Proficiency with electronic health record systems and Microsoft Office.
? Strong communication skills, both written and verbal.
? Ability to interact effectively with funding sources and referral agencies.
? Skilled in problem-solving, prioritizing tasks, meeting deadlines, multitasking, and working independently.
Education & Experience
? Bachelor's degree in counseling, criminal justice, or a related field.
? Experience working within the criminal justice environment.
? Successful completion of introductory employment requirements

Ethical, self-motivated, dependable, reliable, and detail oriented.
? Able to work effectively with clients, staff, and management.
? Capable of meeting routine physical demands (e.g., climbing stairs, carrying up to 20 lbs.).
? Sufficient dexterity and vision to operate office equipment.
? You must possess a valid driver's license.
