Intake Coordinator/Case Manager

Summary:

  • The Intake Coordinator/Case Manager position is responsible for the coordination and management client direct care services. The Intake Coordinator/ Case Manager will provide programmatic information clients and external customers. The Intake Coordinator/Case Manager will be responsible for developing and reviewing appropriate and timely documentation for all services provided. The Intake Coordinator/Case Manager will be in compliance with all guidelines imposed by the Department of Corrections.

Essential Duties and Responsibilities:

  • Provide all Intake duties including: Initial Intake; Assessments; Referrals; Evaluations; PREA documentation; Bed assignments; Census (population) sheets; Client handbook; orientation to program structure and Consents
  • Complete Entitlement documentation for procurement of Medicaid.
  • Provide and document all case management duties including: Re-Entry Plans, Re-Entry Plan Reviews, Progress Notes, Fee Collection and Documentation; Client schedules; Employment verifications; individual counseling and evaluations.
  • Provide client direct care services including: facility head counts and tours; monitoring of self administration of medication; client accountability checks and facility/grounds safety.
  • Completion of mandatory trainings
  • Knowledge and understanding of all Department of Corrections contractual agreements in accordance with the Residential Provider Manual

Required Knowledge, Skills, Abilities and Other Characteristics:

Knowledge of:

  • Electronic Healthcare Record systems. ·
  • General office software, particularly Microsoft Office Suite

Ability to:

  • Interact effectively with representative of funding sources and referral agencies.
  • Analyze and solve problems effectively and efficiently.
  • Communicate effectively both verbally and in writing.
  • Manage priorities to meet deadlines and demonstrate multi-tasking skills.
  • Work and make decisions independently.
  • Assimilate new information and technology.

Education and Experience:

  • Bachelors Degree with a concentration in the area of Counseling or Criminal Justice
  • Experience within the criminal justice environment

Must be:

  • ethical, self-motivated, dependable/reliable and detailed oriented. ·
  • Must be able to interface easily with clients, staff, and other management. ·
  • Must be able to meet routine office physical demands including climbing stairs, carrying up to 20 pounds objects and possessing the dexterity and vision to operate office computers and equipment. · *
  • Must possess a Valid driver's license
